Pick the right assets to analyse and define clean physical and functional boundaries before you spend any effort on FMEA.
Define Functions & Performance Standards
Answer RCM Question 1: what is this asset supposed to do, and to what standard, in its current operating context?
Identify Functional Failures
Answer RCM Question 2: in what ways can each function fail to deliver its performance standard?
Failure Modes & Effects (FMEA)
Answer RCM Questions 3 & 4: what causes each functional failure, and what happens when it occurs?
Failure Consequences
Answer RCM Question 5: does this failure matter? Classify every mode as Hidden, Safety/Environmental, Operational or Non-operational.
Task Selection (Decision Logic)
Answer RCM Questions 6 & 7: what proactive task, if any, is technically feasible and worth doing — and if none, what default action?
Implement, Live & Continuously Improve
Load the accepted tasks into the CMMS, execute them, measure the reliability outcome and re-run the analysis when the context changes.
